Management of Small Intestinal Obstruction


Abstract
139 patients with small intestinal obstruction treated at the surgical service of Seoul National University Hospital between January 1976 and December 1980 were reviewed and the followings were concluded.
1. The major cause of the small intestinal obstruction was postoperative adhesions (68.3%) and the hernias were negligible (1.4%) as the cause of it.
2. Gas-in-colon on the plain abdominal films was helpful to distinguish incomplete obstruct-ions from complete ones.
3. The more the arrival after the onset of symptoms delayed, the higher the chance of small intestinal resection were.
4. 68% of the patients with conservative treatment were improved symptomatically within 48 hours and the delayed decision on surgery had a high chance to make the patients undergo intestinal resection.
5. Postoperative complications and mortality were higher in -patients with resection than in those with simple lysis of the adhesion.
6. The patients with resection of more than 100cm of the small intestine revealed nutritional disturbances such as diarrhea, weight loss and anemia.
7. 68.05 of the patients. improved after conservative treatment and 14.3% of those under.-went surgery developed symptoms again during the period of follow-up.
